Either during or after our consultation call, we can schedule an appointment for a full evaluation. If we don't decide on a time during the call, you can book the appointment through the electronic health record portal. To do this, you will need the login information I send you in a separate email. To use our time well, you will need to complete the intake forms prior to the first appointment. If you are having any difficulty accessing the forms, contact me beforehand.
